Once done, you may then proceed in filling out another form. You may also request for a criminal background check PA by downloading the SP4-164 form and send it by mail. All you need to do is just fill out the form completely. The form includes your name, address, contact information, the reason for your request and the personal information of the individual whom you would like to make a request. Once done, you may prepare for the payment in the form of check or money order. Checks and a money order should be made payable to Commonwealth of Pennsylvania.
